We ask for your name and other individually identifying information when you register for Patient Online Portal. We may also access information about you from your provider in order to provide Patient Online Portal services. For example, we access the list of providers you have seen in the past, in order to allow you to select a provider for a new appointment.

HealthCare Partners will not, under any circumstances, share any health or personal information with any third party other than your designated provider.

We have implemented technology and security policies, rules and measures to protect the health and personal data that we have under our control from unauthorized access, improper use, alteration, unlawful or accidental destruction, and accidental loss.
